Bariatric Assessments
Test anxiety is a real thing! Please know that you have nothing to fear here. Preparing for bariatric surgery can be overwhelming. Our team is here to make your journey easier, less confusing, and to help set you up for success.
To help you feel more confident about the process, we have laid out each step along the way. To get started, click on Step One.
FORM. Complete and submit the form below.
PAPERWORK. When you submit the form, we will send you the new patient paperwork associated with a pre-bariatric surgery assessment. These forms typically take between 60-90 minutes to complete.
BOOK. Once you complete and return your paperwork through our HIPAA compliant patient portal, we will send you a scheduling link. At your convenience, you can use the link to make an appointment for your assessment.
VIRTUAL MEETING. You will meet with one of our team members
REPORT. rovider will write a brief report with the recommendations and share it directly with your surgical team.We will make recommendations your surgical team to ensure that you have the right support in place before and/or after surgery.
FREE CONSULT. othing in the report will be a secret or a surprise to you. Your provider will openly discuss the evaluation findings with you during your meeting. After all, this meeting is designed to help you feel prepared for surgery and your post-surgery life. In the rare case that we do not recommend you for surgery, you will know why and we will work together to formulate a plan to get you ready.
Ongoing support
We offer several types of individual therapy appointments, either in person or online, that may help you in your pre- or post-surgery life. Depending on your experiences both before and after surgery, you may find that you need support to work on patterns of emotional eating, difficult body image concerns, coping with past experiences, or navigating how your relationships change as you and your weight change over time.
We offer 30-minute, 50-minute, or 90-minute appointments for individual therapy.

Once you have scheduled your evaluation appointment, you will be asked to complete some questionnaires, a consent form, and other standard privacy agreements online, using our HIPAA compliant platform. These forms typically take 30-minutes or more to complete, and they are completed prior to your face-to-face in-person or virtual meeting. These forms are important and help us determine how to best guide our meeting, so please take your time with them.
2. Your 'face-to-face'
appointment
Next, you will meet with a team member for a 60-minute evaluation appointment. We can see you in person in our Washington DC office or virtually, using our HIPAA compliant telehealth system. This meeting is not a test to be passed and there are no right or wrong answers. The meeting is an opportunity to discuss what is important to you and what you see as your possible barriers to post-surgery life. We will make recommendations to you and your surgical team to ensure that you have the right support in place before and/or after surgery.
